JUNIOR MEMBERSHIP WAIVER & PARENT AGREEMENT

Hammersmith Martial Arts Club

I confirm that I am the parent or legal guardian of the child named in this registration and that I am over 18 years of age. I understand what I am signing and enter into this agreement voluntarily on behalf of my child and my family.

I understand that Hammersmith Martial Arts Club is a full-contact martial arts academy teaching Brazilian Jiu-Jitsu, Wrestling, Judo, K1 Kickboxing, MMA and related physical activities.

I understand that these activities involve physical contact and carry inherent risks including injury, illness, and in rare cases serious injury. I give permission for my child to participate and accept these risks.

Nothing in this agreement excludes liability for death or personal injury caused by negligence where such liability cannot be excluded under UK law. However, I acknowledge the inherent risks of participation and agree to the terms below.

MEDICAL AND BEHAVIOURAL DISCLOSURE

I agree that I will inform the club before training begins of any medical, physical, learning, emotional, or behavioural conditions that may affect my child’s participation, safety, or interaction with others. This includes (but is not limited to):

• injuries or physical conditions

• asthma or medical conditions

• medications

• neurodiversity or behavioural needs

• sensory or emotional regulation issues

I understand that failure to disclose relevant information may affect my child’s safety and the safety of others.

I confirm that:

• my child is fit to participate, or

• I have taken appropriate medical advice and still choose for them to participate

I agree not to send my child to training if they are unwell, injured in a way that makes training unsafe, or contagious.

ASSUMPTION OF RISK

I understand that martial arts training includes:

• grappling and takedowns

• throws and groundwork

• controlled sparring

• physical exertion

• risk of falls or collisions

These activities may result in:

• bruising

• strains

• sprains

• broken bones

• concussion

• illness

I voluntarily allow my child to participate and accept the inherent risks involved.

PARENT/GUARDIAN RESPONSIBILITIES

I agree that:

• I will ensure my child behaves respectfully and follows all club rules

• I will ensure my child arrives and is collected on time

• I remain responsible for my child outside of class times

• instructors cannot supervise children outside scheduled sessions

I understand that the club is not responsible for children before or after class unless they are in a supervised session.

RULES AND CONDUCT

I understand that martial arts training requires discipline, safety awareness, and respect.

If my child behaves in a way that:

• endangers others

• disrupts classes

• damages equipment

• fails to follow instructions

the club may remove them from training.

I agree to support instructors in maintaining safe and respectful training environments.

I accept responsibility for damage caused by my child to facilities or equipment and agree to pay reasonable repair or replacement costs.

RELEASE AND LIABILITY

To the fullest extent permitted by law, I acknowledge that participation in martial arts training carries inherent risks and I accept those risks on behalf of my child.

I understand that while the club will take reasonable care in instruction and supervision, injuries may still occur as part of normal training.

I agree not to hold the club responsible for injuries arising from the inherent risks of martial arts training except where liability cannot legally be excluded under UK law.

MEMBERSHIP STATUS AND RIGHT TO TERMINATE

I understand that Hammersmith Martial Arts Club operates as a private members club and that membership is a privilege, not a right.

The Club reserves the absolute right, at its sole discretion and without the requirement to provide a reason, to:

• refuse membership

• suspend membership

• terminate a child’s membership

• remove a child or parent from classes or premises

This may occur where the Club considers it necessary for safety, behaviour, welfare, or the culture and operation of the academy.

I understand and agree that:

• membership may be terminated at any time

• no refund is guaranteed if membership is terminated

• I will comply with any request for my child or myself to leave the premises

• termination does not create liability for the Club or its staff

Any such decision by the Club is final.

MEMBERSHIP CANCELLATION NOTICE

I understand that membership is ongoing and will continue until cancelled.

If I wish to cancel my child’s membership, I agree to provide a minimum of one calendar month’s notice in writing.

Membership fees remain payable during the notice period.

ILLNESS AND HYGIENE

I understand that training involves close physical contact and may expose participants to illness or viruses. I accept this risk and confirm that I will not allow my child to train while unwell or contagious.

MEDIA CONSENT

I understand that classes or events may be photographed or filmed for promotional or educational purposes and I consent to my child’s image being used for these purposes without compensation.

DATA SHARING

I understand that my child’s details may be shared with relevant governing bodies where necessary for membership, grading, or competition entry.

ENFORCEABILITY

If any part of this agreement is found to be unenforceable, all remaining parts shall remain in full effect.

UNDERSTANDING OF AGREEMENT

I confirm that:

• I have read and understood this agreement

• I have had the opportunity to ask questions

• I sign voluntarily as parent/guardian

• I agree to be bound by its terms

I understand that martial arts training involves risk and I consent to my child’s participation.
